Services for Seniors and Caregivers

San Joaquin County
Caregivers Respite
We assist caregivers of the frail, elderly seniors suffering physical and or mental disabilities (Alzheimer's Disease, senile dementia, or those who are bedridden and should not be left unattended). We provide a Respite Care Worker to stay with the dependent senior so that his/her caregiver can take a temporary break from the caregiving situation. Respite care services are provided directly in the client's home.
Golden Agers for Progress (GAP)
Through research,
education, action and advocacy, seniors from all walks of life join in
a common goal to “mind the GAP” and narrow the disparity in services
versus the numbers who need them. “Minding the GAP” is the focus of
seniors doing something to make improvements for themselves and other
seniors. They are “seniors helping seniors”.

Homemaker Services
This program provides services to seniors who are in need of assistance with housekeeping (light cleaning, cooking, laundry).

Home - Modification Program for Caregivers
The purpose of this program is to facilitate safety and to ease the task of caregiving in the home, by providing the caregiver with practical tips and assistance on setting up the care recipient's living environment, making minor modifications to the home and by supplying and installing various adaptive equipment which would help the caregiver do his/her job just a little easier.

Kaiser THRIVE Program
The Kaiser THRIVE Program provides in-home services (housecleaning, personal care), information, and support to non-English speaking seniors and seniors who cannot easily access needed services such as healthcare, case management, etc. because of the remoteness of where they live. We will serve 25-30 seniors.
Multipurpose Senior Services Program (MSSP)
The Multipurpose Senior Services Program provides care management of social and health care services for frail elderly, in an effort to keep them independent and living in their own homes. MSSP is a cost effective alternative to the institutionalization of the frail elderly.
Personal Care
We provide non-medical assistance to seniors who need help with personal care (bathing, dressing, shaving, toileting). We do not provide nursing care (wound care, glucose testing, incontinency care or administration of medications). Clients must be able to lift or transfer themselves independently or with only minimal assistance. In addition, transportation to medical appointments, grocery shopping and even hair appointments is available.

PG&E "Calling All Seniors" Telephone Reassurance and Friendly Visiting
We provide services to seniors to reduce loneliness and isolation by giving assurance and support through telephone calls by trained volunteers. This service assists seniors in maintaining their independence and quality of life by addressing their emotional needs through regular communication.

Behavioral Health Services
As an extension of the Bishop’s Social Justice Ministry, the Catholic Charities Behavioral Health Program provides behavioral health outreach and engagement to elderly and dependent adults and family members residing in long-term care facilities and the community.

Elder Abuse Prevention Program
The Elder Abuse Prevention Program was created to educate the public about the physical, emotional and financial abuse of our seniors. Outreach efforts include presentations and training sessions for Certified Nursing Assistants, facility staff, clergy, service organizations, financial institutions and other mandated reporters. Topics include mandated reporting procedures, identity theft, scams and other abusive situations which victimize seniors and other dependent adults.

Legal Advocacy Program
Free legal advocacy for adults 60 years and older is provided by the Legal Advocacy Program. Services include assistance with wills, landlord or renter issues, problems with contractors, fraud, and other legal issues.

Ombudsman Program
The Long-Term Care Ombudsman Program advocates for the rights and quality care for residents of skilled nursing facilities, residential care facilities for the elderly, and those who attend adult day health care programs.
Senior Options
Senior Options is a case management program supporting the senior’s desire to remain independent and in their own homes and communities. The case manager identifies needs, helps the individual access direct services, and provides education and advocacy. Over 450 seniors in Stanislaus County have received services through this important program.

Social Security Representative Payee Program
As a Representative Payee Organization, we receive monthly SSA/SSI benefits on behalf of seniors who are unable to manage their money. This money is used only to pay for the senior's current needs, including housing, utilities bills, food, and personal care items.

Transportation Program
Transportation Program provides seniors 60 years and over transportation to medical appointments and grocery shopping. There are three vans, one of which is equipped with a wheel chair lift. We have two full-time drivers and a number of volunteer drivers who help to make this program possible.
