Senior Options - Stanislaus
Senior Options is a case management program supporting one’s desire to remain independent by identifying needs, accessing or arranging for direct services, and encouraging mobility, education, and advocacy.
The goal of Senior Options is to help older adults remain in their own homes and communities, continuing to live fulfilling lives. With assistance provided by case managers, seniors are often able to avoid or delay the need for institutional living.
Case management services are provided by both Catholic Charities’ employees and (during some semesters), students enrolled in the graduate degree social work program through California State University Stanislaus. Senior Options is based in the Modesto office.
The many collaborative relationships that Catholic Charities has with public and private agencies throughout the region are instrumental in both the identification of clients and arrangement of services. Many Senior Option referrals come from county agencies including Adult Protective Services.
Senior Options was originally launched with a generous grant from the California Endowment. Program funding has also been provided through the Catholic Social Services Guild, Kaiser Permanente, and the Stanislaus Community Foundation. Current funding limitations have required a significant reduction in services while new funding sources are being developed.
When fully operational, our services include: In home assessments – We can help determine social, home safety, health, financial, transportation and other needs. Referral and advocacy – We will make referrals to community agencies and resources to address concerns and work toward long term solutions.
Education – Many options exist. Some areas of concern we provide information on are; fall prevention, awareness of preventable diseases, and home safety.
